Hammersmith & Fulham Council is seeking a dynamic and experienced Occupational Therapist (OT) to join their Community Adult Social Care team. This is a fast-paced project aimed at clearing the waiting list backlog, and we're looking for someone who is fit, motivated, and ready to hit the ground running.

This role involves visiting clients in the community, conducting high-quality assessments, and delivering swift, effective OT interventions that support independence, reduce care needs, and improve outcomes.

Key Responsibilities:
Complete timely and comprehensive assessments in the community, managing a high-volume caseload

Identify and recommend appropriate equipment and major adaptations to support daily living

Deliver moving and handling assessments and recommendations in line with best practice

Review and reduce care packages through enabling interventions

Work collaboratively with service users, carers, and multi-disciplinary teams to achieve positive, person-centred outcomes

Order equipment and liaise with suppliers confidently and efficiently

Maintain clear, concise documentation and adhere to statutory and service standards

Requirements:
HCPC-registered Occupational Therapist

Proven experience in community-based adult social care

Strong working knowledge of equipment ordering, adaptations, and care package reviews

Skilled in moving & handling, manual handling risk assessments, and major adaptations

Ability to manage a fast-paced, high-turnover caseload

Physically fit and able to work out in the community as required

Self-motivated and able to work both independently and as part of a team
