As an Adult's Social Worker, your key responsibilities will include:
- Conducting comprehensive assessments of adults in need of support and safeguarding.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Ensuring compliance with legal and regulatory frameworks, including the Care Act and safeguarding procedures.
- Working closely with multidisciplinary teams and external agencies to provide holistic support.
- Monitoring and reviewing care plans to ensure effectiveness and adjust as necessary.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Demonstrated experience in adult social work, particularly in assessment and safeguarding.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Proficiency in case management systems and documentation.
