Adults Social Worker - Hospital Team (Transfer of Care Hub)
Warrington Hospital
£30 per hour
Full-time (37 hours per week)

Warrington Borough Council is seeking 2 experienced Adults Social Workers to join the Transfer of Care Hub at Warrington Hospital.

This is a fast-paced hospital discharge role supporting safe and timely discharges, working closely with health and multidisciplinary teams to reduce delayed transfers of care.

Key responsibilities:

Support hospital discharge planning and coordination
Undertake Care Act assessments and reviews
Work closely with health and MDT partners
Reduce delayed transfers of care through timely interventions
Ensure safe and appropriate discharge pathways

Requirements:

Recent experience in hospital or Transfer of Care settings
Strong assessment, communication, and decision-making skills
Knowledge of Mosaic system desirable
Qualified Social Worker with SWE registration

Working pattern:

Full-time, 37 hours per week
Hybrid role with approximately 3 days per week office-based at hospital
