Warrington - Adults Social Worker - Hospital Team
Warrington Council is seeking a dedicated and compassionate Adults Social Worker to join our Hospital Team. This role is pivotal in providing high-quality support and intervention for adults in a hospital setting, ensuring their needs are met during a critical time in their lives. As a key member of our multidisciplinary team, you will work collaboratively with healthcare professionals to assess, plan, and implement care plans that promote the well-being and independence of our clients.
Key responsibilities include:
- Conducting thorough assessments of the social, emotional, and practical needs of adults admitted to the hospital.
- Developing and implementing individualized care plans in collaboration with clients, families, and other professionals.
- Providing information and support to clients and their families regarding available services and resources.
- Facilitating discharge planning and ensuring smooth transitions to community services or residential placements.
- Maintaining accurate and up-to-date records in compliance with regulatory standards and organizational policies.
The ideal candidate will possess the following qualifications and skills:
- A relevant social work qualification (Degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience in adult social work, preferably within a hospital or healthcare setting.
- Strong assessment and analytical skills with the ability to make sound decisions under pressure.
- Excellent communication and interpersonal skills to build rapport with clients and work effectively in a team.
