Social worker - Acute Hospital Team - Taunton, Somerset
Social Workers required for work within the Adult Acute Hospital Discharge Team in the Taunton area of Somerset. Excellent rates of pay up to £38ph. 3 month minimum contract but possibility to extend. Exciting opportunity in a lovely area with good transport links from M5 for neighbouring authorities. Flexible hybrid working arrangements.
This role is focused on ensuring safe, timely, and effective discharges from hospital, working closely with health professionals, patients, and families.
Key Responsibilities
* Support and coordinate hospital discharges
* Complete Care Act assessments and reviews
* Work collaboratively with multidisciplinary teams
* Safeguard vulnerable adults and promote independence
* Ensure timely documentation and effective handovers
* Liaise with families, health colleagues, and community services
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
