Adult Social Worker - Adults, Health Partnerships & Housing

Eden Brown Synergy is currently recruiting for an experienced Adult Social Worker to join the Adults, Health Partnerships & Housing team at North Northamptonshire Council.

This is an excellent opportunity to work within a supportive multidisciplinary team, delivering person-centred, outcomes-focused services to adults with care and support needs.

Location

  • William Knibb Centre, Alfred Street, Kettering, NN16 0SW
  • Swanspool House, Wellingborough, NN8 1BP

Working Pattern: Office-based during induction, followed by hybrid working once established in the role.

Key Responsibilities

  • Undertake Care Act assessments, reviews and risk assessments for adults and carers.
  • Develop personalised support plans and work alongside brokers to arrange appropriate care packages, equipment and adaptations.
  • Manage an allocated caseload in line with statutory requirements, professional standards and local policies.
  • Work collaboratively within a multidisciplinary team to achieve positive outcomes for service users.
  • Apply relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act, where appropriate.
  • Maintain accurate case records, reports and documentation.
  • Safeguard vulnerable adults and work effectively with partner agencies, NHS professionals and third-sector organisations.
  • Support and mentor less experienced colleagues where required.

Requirements

  • Degree or equivalent qualification in Social Work (CQSW, DipSW or equivalent).
  • Current registration with Social Work England.
  • Experience working within Adult Social Care, including Mental Health, Older People, Learning Disabilities, Physical Disabilities or Transitions.
  • Strong assessment, case management and risk assessment skills.
  • Excellent communication and organisational skills.
  • Full UK driving licence and the ability to travel independently.
