Adult Social Worker - Adults, Health Partnerships & Housing

£33 - £35 per hour | Hybrid Working | 6-9 Month Contract | 37 Hours per Week
Eden Brown Synergy is currently recruiting for an experienced Adult Social Worker to join the Adults, Health Partnerships & Housing team at North Northamptonshire Council.
This is an excellent opportunity to work within a supportive multidisciplinary team, delivering person-centred, outcomes-focused services to adults with care and support needs.
Location
- William Knibb Centre, Alfred Street, Kettering, NN16 0SW
- Swanspool House, Wellingborough, NN8 1BP
Working Pattern: Office-based during induction, followed by hybrid working once established in the role.
Key Responsibilities
- Undertake Care Act assessments, reviews and risk assessments for adults and carers.
- Develop personalised support plans and work alongside brokers to arrange appropriate care packages, equipment and adaptations.
- Manage an allocated caseload in line with statutory requirements, professional standards and local policies.
- Work collaboratively within a multidisciplinary team to achieve positive outcomes for service users.
- Apply relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act, where appropriate.
- Maintain accurate case records, reports and documentation.
- Safeguard vulnerable adults and work effectively with partner agencies, NHS professionals and third-sector organisations.
- Support and mentor less experienced colleagues where required.
Requirements
- Degree or equivalent qualification in Social Work (CQSW, DipSW or equivalent).
- Current registration with Social Work England.
- Experience working within Adult Social Care, including Mental Health, Older People, Learning Disabilities, Physical Disabilities or Transitions.
- Strong assessment, case management and risk assessment skills.
- Excellent communication and organisational skills.
- Full UK driving licence and the ability to travel independently.
