Eden Brown Synergy is currently recruiting for an experienced Reablement Occupational Therapist to join Slough Borough Council. This is an excellent opportunity to make a real difference by supporting individuals to regain independence and improve their quality of life through a recovery-focused reablement service.

Working Pattern

  • Hybrid working (2 days per week in the office)
  • 37 hours per week
  • Shift-based rota, including weekend cover every 7 weeks

The Role

As a Reablement Occupational Therapist, you will assess, implement and deliver personalised reablement programmes that empower individuals to live safely and independently in their own homes. Working within a multidisciplinary team, you will help service users achieve meaningful outcomes while promoting recovery, wellbeing and independence.

Key Responsibilities

  • Undertake comprehensive occupational therapy assessments.
  • Develop and deliver person-centred, strength-based reablement programmes.
  • Promote recovery, functional improvement and independent living.
  • Support safe and timely hospital discharges.
  • Recommend proportionate long-term care where ongoing needs are identified.
  • Work collaboratively with health and social care professionals within a multidisciplinary team.
  • Maintain accurate case records and ensure interventions comply with relevant legislation and professional standards.
  • Contribute to service development and support continuous improvement initiatives.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience working within reablement, community services, adult social care or hospital discharge settings.
  • Strong assessment, clinical reasoning and care planning skills.
  • Excellent communication and multidisciplinary working abilities.
  • A person-centred approach with a commitment to promoting independence and positive outcomes.
