Assessment Social Worker - Middlesbrough £36.61

We are currently seeking a dedicated and experienced Assessment Social Worker to join our team in Middlesbrough. In this role, you will be responsible for conducting comprehensive assessments of individuals and families in need of social work support, with a focus on safeguarding and promoting their well-being.

Key responsibilities and duties:

  • Undertake assessments to identify needs and risks, and develop appropriate care plans
  • Provide support and guidance to service users, advocating on their behalf where necessary
  • Work collaboratively with multi-disciplinary teams and external agencies to ensure holistic support
  • Conduct home visits and participate in case conferences to review progress and make recommendations
  • Maintain accurate and up-to-date records of all interactions and interventions

Essential qualifications and skills:

  • Qualified Social Worker with relevant professional registration (HCPC)
  • Demonstrated experience in conducting assessments and working with vulnerable populations
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals
  • Sound knowledge of relevant legislation and policies governing social work practice
  • Ability to work independently, manage a complex caseload, and prioritize tasks effectively
