Hackney Council is seeking a Qualified Social Worker to join their Hospital Discharge Team on a locum basis.

About the Role:
You'll be working within a fast-paced hospital discharge environment, supporting adults and their families through safe, timely discharges and ensuring compliance with the Care Act 2014. You will be based in Hackney Service Centre (HSC).

Key Responsibilities:

Undertake assessments, mental capacity decisions, and care planning for hospital discharges

Liaise with health professionals and multi-disciplinary teams to support safe and effective discharges

Develop personalised support plans that meet needs, reduce hospital readmissions, and promote independence

Apply strength-based approaches and safeguard vulnerable adults as necessary
