We are seeking a dedicated and compassionate Adult's Social Support Worker (SSW) to join our Hospital Discharge Team in Dudley. To be successful in this role, you will need to possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
  • Registered with Social Work England.
  • Proven experience in adult social work, particularly in hospital discharge settings.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Excellent problem-solving abilities and a proactive approach to case management.
