Rate: £41 per hour (Umbrella)
Contract: Locum, full-time

Brent Council is seeking an experienced Team Manager / Practice Manager to lead a dynamic Hospital Discharge Team. The role requires a professional who can thrive in a fast-paced environment, ensuring safe, person-centred discharges in line with adult social care legislation.

Key Responsibilities:

Prioritising, screening, and evaluating referrals and funding applications

Overseeing and managing complex casework within a hospital discharge setting

Supervising and supporting a team of social care professionals

Leading on Safeguarding Adults Manager (SAM) activities

Ensuring compliance with the Care Act 2014 and other key legislation

Effective liaison with health professionals and hospital staff to coordinate discharge planning

Essential Requirements:

Qualified Social Worker with active SWE registration

Proven leadership or management experience within hospital discharge settings

Strong knowledge of adult social care frameworks and safeguarding processes

Excellent communication and decision-making skills
