Warrington - QSW - Hospital Discharge Team

Warrington Council is seeking two experienced Qualified Social Workers to join their Hospital Discharge Team, working within the integrated Transfer of Care Hub at Warrington General Hospital.

This is a crucial role supporting timely, safe, and person-centred discharges from hospital settings.

Key Responsibilities:

Carry out Care Act 2014 assessments to facilitate hospital discharge

Work collaboratively within a multi-disciplinary environment

Conduct out-of-area hospital visits as required

Deliver strengths-based, outcome-focused support plans

Coordinate with health colleagues and family members

Prevent delayed discharges and promote safe transitions to community or care settings

Requirements:

Qualified Social Worker with Social Work England registration

Minimum 2 years of statutory Social Work experience (essential)

Proven experience in hospital discharge work or integrated team settings

Access to your own vehicle for travel to other hospitals

Ability to work in a fast-paced, high-pressure environment

Must meet the essential criteria of the JD/PS
