Overview
An NHS Community Mental Health Team in the Telford area is expected to release vacancies shortly for experienced Community Mental Health Nurses to support their Adult CMHT service. These roles will involve managing a community caseload and supporting individuals with complex mental health needs within the community.

The successful candidates will work as part of a multidisciplinary team providing assessment, care planning and ongoing support to service users with severe and enduring mental health conditions.

Key Responsibilities
Manage and coordinate a defined caseload within a Community Mental Health Team.
Undertake comprehensive mental health assessments and risk assessments.
Develop, implement and review care plans in line with CPA processes.
Support crisis management and escalation where required.
Work closely with psychiatrists, psychologists, social workers and other MDT colleagues.
Liaise with external services including primary care, social services and voluntary organisations.
Monitor patient progress and support recovery-focused interventions.
Maintain accurate clinical documentation in line with NHS and NMC standards.

Essential Requirements
Registered Mental Health Nurse with valid NMC registration.
Previous experience working within a Community Mental Health Team (CMHT).
Strong experience managing a community caseload.
Experience with risk assessments, care planning and CPA processes.
Ability to work independently within the community and as part of a multidisciplinary team.

Desirable Experience
Experience within NHS community mental health services.
Knowledge of local authority and community service pathways.
Experience using electronic patient record systems such as RiO or SystmOne.

Recent clinical references
Up to date mandatory training
