Key responsibilities and duties:
- Assessing the needs of adults with learning disabilities and developing care plans
- Providing advocacy and support to individuals in accessing services and resources
- Working collaboratively with multi-disciplinary teams to ensure holistic care
- Conducting safeguarding investigations and taking appropriate actions
- Participating in case conferences and reviews to monitor progress and adjust support as needed
Essential qualifications and skills:
- Qualified Social Worker with relevant experience in adult social care
- Registration with Social Work England
