We are seeking a dedicated and compassionate Adult's Social Worker to join our Dynamic D2A (Discharge to Assess) team in Essex. This pivotal role involves working with adults who require social support and assessment following hospital discharge. You will play a crucial role in ensuring that individuals receive the appropriate care and support to facilitate a smooth transition back into their communities.
Key responsibilities include:
- Conducting comprehensive assessments of clients’ needs and circumstances.
- Developing and implementing care plans in collaboration with clients and their families.
- Providing ongoing support and advocacy for clients to access necessary services.
- Collaborating with multidisciplinary teams, including healthcare professionals and community organizations.
- Maintaining accurate and timely documentation in accordance with organizational policies and legal requirements.
To be considered for this position, you must possess the following qualifications and skills:
- A relevant Social Work qualification (Degree in Social Work or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in a hospital or community setting.
- Strong assessment and analytical skills.
- Excellent communication and interpersonal abilities.
