Adult's Lead - Transition Team - Hammersmith & Fulham
We are seeking a dedicated and experienced Adult's Lead for our Transition Team in Hammersmith & Fulham. This pivotal role involves leading a team of social workers committed to supporting adults with complex needs as they transition into adulthood. You will play a vital role in ensuring that individuals receive the best possible care and support, promoting their independence and well-being while navigating the challenges of adult life.
Key responsibilities include:
- Leading and managing a team of social workers, ensuring high-quality service delivery.
- Conducting assessments and developing tailored support plans for individuals transitioning into adulthood.
- Collaborating with multidisciplinary teams, external agencies, and families to facilitate seamless transitions.
- Providing supervision, guidance, and professional development opportunities for team members.
- Monitoring and evaluating service effectiveness, implementing improvements where necessary.
To be considered for this role, candidates must possess the following qualifications and skills:
- A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
- Registered with Social Work England.
- Significant experience in adult social work, particularly in transition services.
- Strong leadership and team management skills.
- Excellent communication and interpersonal abilities.
- Sound knowledge of relevant legislation and best practices in adult social care.
