We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Transition Team in Bristol. This pivotal role involves supporting individuals as they navigate significant life transitions, ensuring they receive the necessary resources and guidance to enhance their quality of life. As part of our dynamic team, you will work closely with adults who require assistance in adapting to new circumstances, promoting independence and well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and strengths of individuals.
- Developing and implementing tailored care plans that support individual goals and aspirations.
- Collaborating with multidisciplinary teams and external agencies to facilitate seamless transitions.
- Providing ongoing support and advocacy for clients and their families throughout the transition process.
- Maintaining accurate records and documentation in compliance with regulatory standards.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, CQSW).
- Registered with Social Work England.
- Experience working with adults in a social work capacity, particularly in transition settings.
- Strong communication and interpersonal skills, with the ability to build rapport with clients.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
