  • Conducting comprehensive assessments to identify the needs and preferences of service users.
  • Developing, implementing, and reviewing person-centered care plans in collaboration with service users and their families.
  • Providing ongoing support and guidance to clients, ensuring they have access to appropriate resources and services.
  • Working collaboratively with multi-disciplinary teams, including healthcare professionals, to ensure holistic care delivery.
  • Maintaining accurate and up-to-date case records in compliance with regulatory standards.

To be considered for this position, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current Social Work England registration.
  • Experience working with adults with physical disabilities and older people.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate approach to service delivery.
