We are seeking a dedicated and compassionate Adult's Social Worker to join our Learning Disabilities (LD) Team in Leeds. Your key responsibilities will include:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences.
- Developing and implementing personalized care plans that promote independence and enhance quality of life.
- Collaborating with multidisciplinary teams, including health professionals and community organizations, to ensure holistic support.
- Providing direct support and guidance to individuals and their families, empowering them to make informed choices.
- Maintaining accurate and up-to-date case records in compliance with statutory requirements and organizational policies.
To be considered for this role, you should possess the following essential qualifications and skills:
- A recognized social work qualification (BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with adults with learning disabilities or in a similar social work setting.
- Strong understanding of relevant legislation and best practices in social work.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust.
