Adult's SW - LD Team - Warwickshire
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities Team in Warwickshire. As a key member of the team, you will be responsible for providing high-quality support and intervention to adults with learning disabilities to promote their independence and well-being.
Key responsibilities and duties include:
- Conducting assessments and developing care plans tailored to the individual needs of service users.
- Working collaboratively with multi-disciplinary teams and external agencies to ensure holistic support.
- Advocating for the rights and best interests of adults with learning disabilities.
- Providing crisis intervention and safeguarding vulnerable adults.
- Participating in case conferences and reviews to monitor progress and adjust support as needed.
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration.
- Demonstrated experience working with adults with learning disabilities in a social work setting.
- Strong assessment and care planning skills with a person-centered approach.
- Excellent communication and interpersonal skills to engage effectively with service users, families, and stakeholders.
- Ability to work autonomously and as part of a team in a fast-paced environment.
In addition to the above requirements, candidates with experience in mental capacity assessments, MCA and DoLS applications, and knowledge of relevant legislation and policies will be highly regarded.
