Adult's SW - Hospital Discharge Team - Bristol

We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Discharge Team in Bristol. As a key member of the team, you will be responsible for facilitating safe and timely discharges for adult patients, ensuring they receive appropriate support and services upon leaving the hospital.

Key responsibilities and duties include:

  • Conducting assessments of patients' needs and developing care plans in collaboration with healthcare professionals and service users.
  • Coordinating with external agencies and community resources to arrange necessary support services for patients post-discharge.
  • Advocating for patients' rights and ensuring their voices are heard in the discharge planning process.
  • Providing emotional support and guidance to patients and their families during the transition from hospital to home or alternative care settings.
  • Completing all necessary documentation and reports in a timely and accurate manner to ensure compliance with regulatory requirements.

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with Social Work England.
  • Demonstrated experience working with adult populations in a hospital or healthcare setting.
  • Strong assessment and care planning skills, with the ability to make sound decisions in complex situations.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams and external partners.
  • Knowledge of relevant legislation and policies related to adult social care and safeguarding.

In addition to the essential qualifications and skills, candidates with experience in discharge planning, mental health, or working with vulnerable adults will be highly valued.
