We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Newcastle in the role of Continuing Health Care. As a key member of our team, you will be responsible for providing high-quality social work services to individuals requiring continuing health care support.

Key responsibilities and duties include:

  • Conducting assessments to determine the ongoing health care needs of service users
  • Developing and implementing care plans in collaboration with multidisciplinary teams

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience in working with individuals with complex health care needs
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively within a multidisciplinary team
