Safeguarding/Care planning - AP - Middlesbrough
We are currently seeking a dedicated and experienced Safeguarding/Care planning Assistant Practitioner to join our team in Middlesbrough. As a key member of our Qualified Social Work field, you will play a crucial role in ensuring the safety and well-being of vulnerable individuals in our community.
Key responsibilities and duties:
- Conducting assessments to identify safeguarding concerns and develop care plans
- Working collaboratively with social workers and other professionals to provide support and intervention
- Monitoring and reviewing care plans to ensure they are effective and meeting the needs of service users
- Participating in case conferences and meetings to contribute to decision-making processes
- Keeping accurate records and documentation in line with regulatory requirements
Essential qualifications and skills:
- A relevant qualification in social work or a related field
- Experience working in a safeguarding or care planning role within a social care setting
- Strong communication and interpersonal skills to engage effectively with service users and stakeholders
- Ability to work autonomously and as part of a multidisciplinary team
- Understanding of relevant legislation and policies related to safeguarding and care planning
