Adult's SW - LD Team - Bath and North East Somerset
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities Team in Bath and North East Somerset. As a key member of the team, you will be responsible for providing high-quality social work services to adults with learning disabilities, empowering them to live fulfilling and independent lives.
Key responsibilities and duties include:
- Conducting assessments and reviews to identify the needs of individuals with learning disabilities
- Developing and implementing care plans that promote independence and well-being
- Working collaboratively with multi-disciplinary teams and external partners to ensure holistic support for service users
- Advocating for the rights and choices of individuals with learning disabilities
- Participating in safeguarding procedures and promoting a culture of safety and protection
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with a valid HCPC registration
- Demonstrated experience in working with adults with learning disabilities in a social work setting
- Strong assessment, communication, and interpersonal skills
- Ability to work autonomously and as part of a team in a fast-paced environment
- Knowledge of relevant legislation, policies, and best practices in adult social care
