We are currently recruiting experienced Qualified Social Workers to join the Transitions Team in Sefton.

This is a vital role focused on ensuring a smooth and coordinated transition for young people moving from children's services into adult services. The additional capacity is urgently required to help meet our statutory duties and maintain continuity of care.

Key Responsibilities:

Support young people aged 14-25 transitioning from childrens to adult services.

Liaise with health, education, and adult care services to ensure person-centred plans.

Undertake assessments in line with statutory frameworks.

Work closely with families, carers, and multi-disciplinary teams.

Requirements:

Qualified Social Worker (SWE Registered)

Experience in transitions or similar work with young adults

Ability to manage complex cases independently

Enhanced DBS on the update service (or willingness to obtain one)
