Location: Bracknell
Rate: £39.95 per hour
Contract: Locum

Working Pattern: Full-time - Hybrid (2-3 days hospital/office based, depending on workload)

Overview:
Bracknell Forest Council is seeking an experienced Qualified Social Worker to join their Hospital Discharge Team, supporting adults through the discharge process and ensuring appropriate follow-up in the community. This is a crucial frontline role that offers a rewarding mix of hospital and office-based work.

Key Details:
2-3 days office/hospital-based, depending on business need

Hybrid working pattern

Managing hospital discharge cases under the Care Act

Contributing to multi-disciplinary planning with health professionals

Ideal for local candidates - must have access to own vehicle

Requirements:
SWE registered Qualified Social Worker

Previous experience in a hospital or discharge team

Strong understanding of the Care Act and discharge processes

Ability to work at pace and manage caseloads effectively

Driving license and own vehicle required
