Job Title: Qualified Social Worker - Hospital Discharge
Location: Portsmouth
Pay Rate: £36.01 per hour
We are currently recruiting for a Qualified Social Worker (QSW) to join the Hospital Discharge team in Portsmouth.
You will be mainly based on _The Limes_ ward, which is located within St James Hospital - Milton. In this role, you will be responsible for managing a caseload of patients and their discharge. This could involve community options or sourcing placements.
Requirements:
A driving license and own vehicle are required for this role.
You will be supporting placement visits in the community, which are not always within the Portsmouth area.
