Manchester City Council is seeking an experienced and proactive Qualified Social Worker to join the Central Hospital Discharge to Assess (HDTA) Community Team. This integrated team provides vital support to citizens through the hospital discharge process, ensuring full assessments are completed in the community and in appropriate settings.

This is a fast-paced, flexible role requiring confident, skilled practitioners with a strong understanding of social work within multidisciplinary environments, including D2A, crisis response, intermediate care, and hospital settings.

Key Responsibilities:

Conduct strength-based assessments in the community following hospital discharge

Work collaboratively within a multidisciplinary team

Support citizens across the full discharge pathway, from hospital to community

Contribute to crisis response and intermediate care pathways as required

Participate in the team's 7-day rota (1 weekend in every 7)

Requirements:

Social Work qualification and registration with Social Work England

Enhanced DBS clearance

Medical clearance (PPHA)

Full, clean driving licence and access to a car

Experience in hospital discharge or community social work

Flexible, responsive, and able to work under pressure
