We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's SW - Review/Extra Care Team in Salford. This role is pivotal in ensuring that the needs of vulnerable adults are met through comprehensive assessments and effective care planning. As part of a dynamic team, you will contribute to the delivery of high-quality support services that empower individuals to live independently and with dignity.

Key responsibilities include:

  • Conducting thorough assessments of individuals’ needs and developing personalized care plans.
  • Reviewing and monitoring existing care packages to ensure they remain effective and appropriate.
  • Collaborating with multidisciplinary teams, including healthcare professionals and community organizations, to provide holistic support.

To be successful in this role, candidates must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social care, particularly in review and assessment processes.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
  • Excellent organizational skills and the ability to manage a caseload effectively.
