Social Worker - Community Team - Southampton
Social Worker required for work within the Adult's Community team in Southampton City Council. Competitive rates of pay rate up to £35ph. 37 hours per week. 3 month minimum contract with options to extend. Flexible, hybrid working arrangements offered.
Southampton is seeking an experienced Community Social Worker to support adults with complex physical needs within the community. This is a rewarding opportunity to work in a person-centred role, promoting independence and improving quality of life for service users.
Key Responsibilities:
Manage a caseload of adults with complex physical needs.
Complete Care Act assessments, reviews, and support planning.
Work closely with health professionals and multi-disciplinary teams.
Support safe hospital discharges and ongoing community care.
Undertake safeguarding enquiries where required.
Promote independence, wellbeing, and positive outcomes.
