Adult's SW - Assessment Team - Liverpool
We are seeking a dedicated and compassionate Qualified Social Worker to join our Assessment Team in Liverpool, focusing on adult services. In this pivotal role, you will be responsible for conducting thorough assessments of adults in need of social care, ensuring that their needs are met with dignity and respect. You will work collaboratively with individuals, families, and other professionals to develop and implement effective care plans that promote independence and well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments of adults to identify their social care needs.
- Developing, implementing, and reviewing care plans in partnership with service users and their families.
- Providing advice, support, and information to individuals and their families regarding available services.
- Collaborating with multidisciplinary teams and external agencies to ensure a holistic approach to care.
- Maintaining accurate records and documentation in accordance with local policies and procedures.
To be successful in this role, candidates must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in assessment and care planning.
- Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations.
- Excellent organizational skills and the ability to manage a caseload effectively.
