Your key responsibilities will include:
- Assessing the needs of children with disabilities and their families to create comprehensive support plans.
- Conducting regular reviews and updates of care plans to ensure effective service delivery.
- Working closely with multi-disciplinary teams, including health professionals, educators, and other social services.
- Providing direct support to families, including advice, advocacy, and crisis intervention.

To be considered for this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registration with Social Work England.
