Doncaster - Social Worker - Adults North Team
We are seeking a dedicated and compassionate Social Worker to join our Adults North Team in Doncaster. This role is vital in providing high-quality support and intervention to adults in need, ensuring their well-being and promoting their independence. As a Social Worker, you will work collaboratively with service users, families, and other professionals to assess needs, develop care plans, and deliver appropriate services.
Key responsibilities include:
- Conducting thorough assessments of individuals' needs, strengths, and risks.
- Developing and implementing person-centered care plans in collaboration with service users.
- Providing ongoing support and advocacy to ensure the best possible outcomes for clients.
- Working closely with multidisciplinary teams and external agencies to coordinate services.
- Maintaining accurate records and documentation in compliance with local and national standards.
Essential qualifications and skills include:
- A recognized social work qualification (BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Experience in adult social work, particularly with vulnerable populations.
- Strong assessment, communication, and interpersonal skills.
- Ability to work independently and as part of a team, managing a caseload effectively.
