Adult's SW - Hospital Team - City of Wolverhampton Council
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Team at the City of Wolverhampton Council. In this role, you will play a crucial part in providing high-quality social work services to adults in hospital settings, ensuring their well-being and supporting their transition back into the community.
Key responsibilities and duties:
- Conducting assessments and developing care plans for adults in hospital, in line with relevant legislation and best practice guidelines.
- Working collaboratively with healthcare professionals, service users, and their families to ensure holistic and person-centered care.
- Advocating for the rights and choices of adults in hospital, promoting independence and empowerment.
- Participating in multi-disciplinary meetings and reviews to coordinate care and support interventions.
- Providing emotional support and crisis intervention as needed, addressing safeguarding concerns promptly and effectively.
Essential qualifications and skills:
- Qualified Social Worker with a relevant social work qualification and HCPC registration.
- Demonstrated experience in adult social work, ideally within a hospital or healthcare setting.
- Strong knowledge of relevant legislation, policies, and procedures related to adult social care.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with service users and stakeholders.
- Effective time management and organizational skills, with the ability to prioritize and manage a complex caseload.
