We are currently seeking a Generic Adult Social Worker to join the Locality West Team at Leicester City Council. This is a hybrid role with flexible working options - a minimum of one day per week in the office is expected, with team meetings held face-to-face every four weeks. Supervisions are preferably in person but can be discussed flexibly.

Key Details:
Hourly Rate: £34 (via umbrella)

Location: West Leicester (hybrid working)

Contracted Hours: 30 per week (flexible working between 08:30 - 17:00)

Duration: Ongoing

About the Role:
You'll work with adults with physical health conditions and older people with dementia, including support for discharges from mental health wards. This team provides longer-term support to individuals receiving adult social care who either reside in the west of Leicester or are registered with GPs in the West Primary Care Network.

Key Responsibilities:
Conduct Care Act assessments, reviews, and support planning

Support hospital discharges and prevent readmissions

Work closely with multi-disciplinary teams and primary care professionals

Deliver person-centred, strengths-based practice

Manage a caseload autonomously while maintaining regular supervision

Requirements:
Qualified Social Worker registered with Social Work England

Experience in adult social care, including dementia and physical health

Ability to work both independently and as part of a team

Strong communication, assessment, and care planning skills

Knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act
