Job Advert: Adults Social Worker - Pathway One Integrated Team
Location: Westminster City Council
Working Pattern: Full-time, hybrid (office-based 3 days per week)
Reports to: Team Manager, Home First Service
About the Role:
Westminster City Council's Pathway One Integrated Team is seeking an experienced Adults Social Worker to join our dynamic and fast-paced team. This is a vital role that supports individuals across the tri-borough area, ensuring timely interventions and high-quality care planning.
The successful candidate will play an integral part in delivering services that promote independence, safeguard vulnerable adults, and support our communitys well-being.
Key Responsibilities:
Conducting urgent home visits in the community.
Completing Care Act Assessments and developing Support Plans.
Updating detailed and accurate case notes.
Undertaking Mental Capacity Assessments and making best-interest decisions.
Collaborating with the Home First Team multidisciplinary team (MDT).
Carrying out safeguarding initial enquiries.
What We Are Looking For: The ideal candidate will:
Have 2+ years of UK post-qualifying experience in a local authority adults' social work setting.
Be experienced in hospital discharge or fast-paced community teams.
Demonstrate strong skills in risk assessment and managing complex cases.
Be proficient in using case management systems, with experience in Mosaic being highly desirable.
Thrive in a collaborative and multidisciplinary environment.
Essential Requirements:
A degree or diploma in Social Work.
Registration with Social Work England.
