As a member of this dynamic team, you will play a crucial role in providing support and assistance to individuals with complex care needs within the Westminster area.
Key responsibilities and duties:
- Conducting assessments and developing care plans for individuals with complex care needs.
- Providing ongoing support and intervention to ensure the well-being and safety of vulnerable individuals.
- Liaising with healthcare professionals, service providers, and other agencies to coordinate care and support services.
- Maintaining accurate and up-to-date case records and reports in compliance with regulatory standards.
- Participating in multidisciplinary team meetings and case conferences to review and evaluate care plans.
Essential qualifications and skills:
- Qualified Social Worker with a recognized social work qualification and registration with the relevant governing body.
