Social Worker - Older People & Hospital Team | Conwy County Borough Council | £36.69/hr
Location: Colwyn Bay, Conwy (Eirias Park Leisure Centre)
Rate: £36.69 per hour
Hours: 37 hours per week, Monday to Friday
Duration: Initial 12 weeks with potential extension
Requirement: Must be local or within commutable distance
Conwy County Borough Council is currently seeking an experienced and compassionate Social Worker to join our Older People & Hospital Social Work Services on a short-term basis.
This role involves delivering a high-quality, person-centred social work service to older people and their families/carers, supporting vulnerable individuals to live independently and ensuring their voices remain at the centre of care planning and decision-making.
Key Responsibilities:
Work directly with older people, carers, and families to assess and meet support needs
Promote independence and positive outcomes through strengths-based practice
Collaborate with hospital and community partners to facilitate safe and timely discharges
Maintain accurate records using digital systems
Requirements:
Qualified Social Worker with a minimum of 3 years' post-qualifying experience
Registered with Social Care Wales
Strong interpersonal skills and ability to work effectively in a team
Confident in using IT systems and electronic case recording tools
