We are currently seeking experienced and proactive Qualified Social Workers to join our Adult Social Care Review Team in Wirral. This is a fantastic opportunity to be part of a specialist team delivering outcome-focused, person-centred care through high-quality reviews and reassessments across the borough.
About the Role
As part of the Review Team, you will:
Carry out reviews and reassessments for individuals recently discharged from hospital, as well as those living in the community.
Work closely with at-risk care homes and domiciliary care providers, ensuring quality and continuity of care.
Collaborate with internal teams including Contracts & Commissioning and Quality Improvement to support service development and risk management.
Apply your knowledge of Care Act legislation and person-centred planning to deliver effective interventions.
Support wider adult social care projects and service improvement initiatives.
About You
We're looking for motivated, adaptable professionals with:
Social Work England registration
A recognised Social Work qualification (DipSW, BA/MA Social Work, or equivalent)
Strong assessment and intervention skills, grounded in legally literate practice
Previous experience working with older adults and understanding of hospital discharge pathways
A commitment to continuous professional development (CPD) and improving outcomes for service users
A UK driving licence and access to a vehicle are desirable due to the community-based nature of the role.
