Overview

We are currently seeking a Band 6 Occupational Therapist to join a busy Transfer of Care Team, supporting both acute and community pathways to ensure safe and timely patient discharge.

Key Responsibilities
Undertake comprehensive occupational therapy assessments for patients with complex physical and psychological needs
Support discharge planning with a focus on Pathway 1 and admission avoidance
Work across acute, community, and inpatient rehabilitation settings
Develop and implement patient-centred treatment plans
Promote independence, rehabilitation, and self-management
Work closely within a multidisciplinary team to improve patient flow and outcomes
Manage a varied and complex caseload autonomously
Requirements
HCPC Registered Occupational Therapist
Previous Band 6 experience (or strong Band 5 stepping up)
Experience within discharge planning / community / rehab settings
Strong MDT working and communication skills
Ability to work independently and manage workload effectively
