Social Worker - Adults Home first team - Dorset
Social Workers required for work within the Adult Homecare First Team based in Sherborne the area of Dorset. Excellent rates of pay up to £35ph. 3 month minimum contract but likely extended. 37 hours per week. Flexible hybrid working arrangements.
Based in Dorset to complete home and hospital visits as needed Sherborne, Dorset County Hospital, Royal Bournemouth Hospital or Poole General You will be required to work flexibly in the community and reduce unnecessary hospital stays.. You would need to have current experience in statutory work with Adults, be fully proficient with MCA and capable of thinking and working independently and at a fast pace. Significant experience of undertaking safeguarding work and assessments under the Care Act and MCA as well as Court of Protection work is a must.
You will be involved in comprehensive assessments and safeguarding work that will identify real outcomes for our customers that will enable them to remain as independent as they are able within their community.
Responsibilities comprise the following:
* Plan, organise and be responsible for case co-ordination and implementation of care & protection plans.
* Working with service users and their families/carers
You will be Social Work England registered as a social worker, ideally a driver with access to own car and able to reside and work in the UK
