- Conducting comprehensive assessments of individuals' needs and strengths.

- Providing ongoing support, advocacy, and guidance to clients, ensuring their voices are heard.
- Working closely with multidisciplinary teams and external agencies to coordinate services and resources.
- Monitoring and reviewing progress, making adjustments to support plans as necessary.
- Maintaining accurate and timely documentation in compliance with regulatory standards.
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with a valid Social Work England registration.
- Experience working with individuals with cognitive and physical disabilities.
- Strong assessment and intervention skills, with a person-centered approach.
- Excellent communication and interpersonal skills, enabling effective collaboration with clients and colleagues.
- Sound knowledge of relevant legislation, policies, and best practices in social work.
