Eden Brown Synergy is currently recruiting for a Qualified Social Worker (Permanent) to join the Ageing Well Adults Community Team within Southend-on-Sea Borough Council.

This is an excellent opportunity for an experienced and motivated Social Worker to join a forward-thinking service focused on promoting independence, wellbeing, and strengths-based practice within adult social care.

About the Role

You will be working within the Ageing Well Service, supporting older adults and adults with mental ill health across Southend-on-Sea (including out-of-borough cases). The role focuses on promoting independence, reducing reliance on long-term care, and enabling individuals to remain within their communities wherever possible.

You will:

  • Undertake strengths-based assessments and care planning
  • Promote independence, resilience, and community-based support
  • Work in partnership with health colleagues, GPs, hospitals, and community organisations
  • Carry out safeguarding enquiries, Mental Capacity Act assessments, and (where required) DoLS work
  • Support individuals to achieve personalised outcomes using a person-centred approach
  • Contribute to innovative and collaborative multi-agency working

Salary & Benefits
