We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Team in Bristol. As a key member of the team, you will be responsible for providing high-quality social work services to adults in hospital settings, ensuring their well-being and promoting their independence.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs and risks of adults in hospital care
  • Developing and implementing care plans in collaboration with multidisciplinary teams
  • Providing support and advocacy to individuals and their families to address social and emotional challenges
  • Facilitating safe and timely discharges by coordinating services and resources
  • Participating in case conferences and reviews to ensure the ongoing effectiveness of interventions

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with HCPC registration
  • Demonstrated experience in adult social work, preferably within a hospital or healthcare setting
  • Strong assessment and analytical skills with the ability to make evidence-based decisions
  • Excellent communication and interpersonal skills to engage with a diverse range of individuals
  • Ability to work effectively under pressure and manage competing priorities

Additional requirements for this role include a valid driver's license and access to a vehicle for travel between hospital sites.
