Bristol - QSW - Through Care Team

We are currently seeking a dedicated and experienced Qualified Social Worker to join our Through Care Team in Bristol. As a key member of the team, you will be responsible for providing high-quality support and intervention to children and young people transitioning out of care.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for children and young people
  • Working collaboratively with other professionals and agencies to ensure holistic support
  • Advocating for the needs and rights of children and young people in care
  • Monitoring and reviewing progress, making adjustments to support plans as needed
  • Providing emotional support and guidance to help individuals navigate the challenges of leaving care

The ideal candidate will possess:

  • A recognized social work qualification and registration with Social Work England
  • Demonstrated experience in working with children and young people in a social work setting
  • Strong assessment and intervention skills, with a focus on promoting positive outcomes
  • Excellent communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work autonomously and as part of a multidisciplinary team
