Bristol - Social Worker - Community Adult Social Care Team
We are currently seeking a dedicated and passionate Social Worker to join our Community Adult Social Care Team in Bristol. As a key member of our team, you will be responsible for providing high-quality social work support to adults in the community, ensuring their well-being and promoting their independence.
Key responsibilities and duties include:
- Conducting assessments and developing care plans for adults in need of support
- Providing advocacy and support to individuals to access appropriate services
- Working collaboratively with other professionals and agencies to coordinate care
- Participating in safeguarding procedures and ensuring the protection of vulnerable adults
- Offering guidance and support to families and carers
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with HCPC registration
- Experience working in adult social care, preferably within a community setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a multidisciplinary team
