Adult's SW - South Community Team - Bristol

We are currently seeking a dedicated Adult's Social Worker to join our South Community Team in Bristol. As a key member of the team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.

Key responsibilities and duties:

  • Conducting assessments to determine the needs of service users
  • Developing and implementing care plans in collaboration with service users and their families
  • Providing support and guidance to service users to promote their well-being and independence
  • Working closely with other professionals and agencies to coordinate services and ensure holistic care
  • Maintaining accurate and up-to-date records of interventions and progress

Essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Demonstrated experience in adult social work, preferably within a community setting
  • Strong assessment and analytical skills to identify needs and risks
  • Excellent communication and interpersonal skills to build rapport with service users and stakeholders
  • Ability to work effectively within a multidisciplinary team and manage a complex caseload
