Adult's SW - LD Team - Lincolnshire
We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult’s Learning Disabilities (LD) Team in Lincolnshire. This role is vital in supporting adults with learning disabilities to lead fulfilling lives and achieve their personal goals. As a member of our dynamic team, you will work collaboratively with individuals, their families, and various stakeholders to deliver person-centered care and interventions.
Key responsibilities include:
- Conducting assessments to identify the needs and preferences of adults with learning disabilities.
- Developing and implementing care plans that promote independence and well-being.
- Providing ongoing support and guidance to individuals and their families.
- Collaborating with healthcare providers, community organizations, and other professionals to ensure holistic care.
- Advocating for the rights and interests of service users within the community.
The ideal candidate will possess essential qualifications and skills, including:
- A recognized social work qualification (e.g., Degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience in working with adults with learning disabilities.
- Strong assessment, communication, and interpersonal skills.
- The ability to work independently and as part of a team.
