  • Developing, implementing, and reviewing person-centered care plans in partnership with clients and their families.
  • Providing ongoing support and advocacy to help clients access necessary services and resources.
  • Facilitating and participating in multidisciplinary team meetings to ensure integrated care delivery.
  • Maintaining accurate and up-to-date case records in compliance with legal and regulatory requirements.

To be successful in this role, you will need to possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Experience working with adults with learning disabilities or related fields.
  • Strong communication and interpersonal skills, with the ability to build effective relationships.
  • Excellent organizational skills and the ability to manage a varied caseload.
