Social Worker - Acute Hospital Discharge
Somerset
£38 per hour

Somerset is seeking an experienced Social Worker - Acute Hospital Discharge to support safe and timely hospital discharges.

The role focuses on working within an acute hospital setting, supporting discharge planning in line with the intermediate care process.

Key requirements:

Qualified Social Worker with SWE registration
Strong knowledge of the Mental Capacity Act
Experience in discharge planning and hospital social work
Ability to work on-site daily

Key responsibilities:

Supporting hospital discharge planning and coordination
Completing Care Act assessments and related documentation
Working closely with health professionals and multi-agency teams
Ensuring safe and appropriate discharge pathways for service users
